POCAPALOOZA | London: The International Board of Perioperative Medicine and the Centre for Perioperative Care

“At the moment perioperative care in the UK doesn’t really have the profile that I think it needs; now I suspect that COVID-19 has raised it up the agenda” If you enjoyed Monty’s explanation regarding the UK’s perioperative journey you may find this link interesting: Evidence submitted by the Improving Surgical Outcomes Group (ISOG) (NICE 75): https://publications.parliament.uk/pa/cm200607/cmselect/cmhealth/503/503we35.htm “Now we’re here at this Pocapalooza” – POC, standing for Perioperative Care and “apalooza” being an extravagant event. However, as one of our guests suggests, perhaps we should call this one a “CPOC-apalooza!” as a lot of our focus is on The CPOC (Centre for Perioperative Care). The International Board of Perioperative Medicine is essential for anyone listening to this podcast: https://www.internationalboardpom.org/ Presented by Desiree Chappell with Monty Mythen, and their guests Mike Grocott – a frequent TopMedTalk anchor – and Professor of Anaesthesia and critical care at the University of Southampton and David Selwin, Medical Director at Sherwood Forest Hospitals NHS Foundation Trust. POCAPALOOZA | Sydney: Education, education, education.

“It also must start and end in primary care” This piece is a fascinating in-depth conversation moving through various medical perspectives on perioperative medicine from Australia and New Zealand. A real highlight of the festival. How has perioperative medicine evolved over the years around the world? What’s the status of Perioperative Care as a specialty? How does one specialise in this area, in terms of education? Discussing the development of the International Board:- “We decided perioperative education should not only be available for developed countries”. We hear that TopMedTalk engaging with the African Perioperative Research Group (APORG) has helped move the cause of perioperative care forward, both in Africa and elsewhere. The conversation further evolves: Once educated how do we demonstrate the value of perioperative practice? How do we launch projects that will really make a difference? Where, from a research perspective, do the more fruitful areas of investigation present themselves? Would a minimum data set help? Moving to clinical trials we hear how research is being conducted, both the challenges and the sources of success. “Perioperative medicine is a team sport” – how do we use the interdisciplinary aspect of what we do to enhance our educational options? Regular listeners to TopMedTalk may recall the series we did about the Perioperative Special Interest Group (SIG) – if you want more details of http://www.anzca.edu.au/fellows/special-interest-groups/perioperative-medicine The ANZCA Clinical Trials Network can be found at http://www.anzca.edu.au/ctn Also, The International Board of Perioperative Medicine is here: https://www.internationalboardpom.org/ Further, make sure you check out https://www.periopmedicine.org.uk/ for more information on the perioperative medicine short course we discuss. POCAPALOOZA | Hong Kong, Mike Irwin

EBPOM might be over but the global perioperative festival, the POCAPALOOZA, continues. Either we’ve locked our team in their rooms and forced them to continue broadcasting or they’re driven by a raw enthusiasm for the results of perioperative care to be shared around the world, either way this piece is our penultimate installment before we return to Cape Town. What’s the situation like, in terms of medicine, in Hong Kong at the moment in the wake of COVID 19? “Early intervention and people taking it seriously is a big thing” The conversation evolves as our guest speaks to the realities of moving internationally to pursue your career in medicine. POCAPALOOZA | EBPOM USA, PASS clinic and shared decision making

The focus for the first part of this POCAPALOOZA was Duke University’s PASS clinic. Find out more about the PASS clinic: https://www.dukehealth.org/locations/duke-pre-anesthesia-testing-center-clinic-2d “So it’s a new approach to really changing the paradigm from just evaluating a patient and sort of passing that information forward to the receiving team […] rather we take a much more proactive progressive approach to identify those chronic co-morbid conditions and try to bend that curve, we try to modify that which may be modifiable” If you’ve heard Sol’s passion and want to transform your practice get more detail from these two excellent pieces from Anesthesia and Analgesia here: Roadmap for Transforming Preoperative Assessment to Preoperative Optimization https://journals.lww.com/anesthesia-analgesia/Abstract/2020/04000/Roadmap_for_Transforming_Preoperative_Assessment.4.aspx?context=FeaturedArticles&collectionId=4 Preoperative Optimization: A Continued Call to Action https://journals.lww.com/anesthesia-analgesia/Citation/2020/04000/Preoperative_Optimization__A_Continued_Call_to.3.aspx There follows an engaging conversation about prehabilitation, “it’s like Rocky Balboa training to get in the ring”. POCAPALOOZA 2020 | Cape Town; The African Surgical Outcomes Study

The POCAPALOOZA story begins with our fascination with The African Surgical Outcomes Study. Over the last year regular listeners have heard key parts of the narrative fall into place during TopMedTalk interviews and discussions. Learn how the European Surgical Outcomes Study (EUSOS), presented at a previous Evidence Based Perioperative Medicine (EBPOM) conference, helped to inspire the African Surgical Outcomes Study ASOS which then went on to inspire us all; after an international gathering in Prato. A truly global community – the perioperative medical community – has now been galvanised into action. Determined to further the research and increase the data at the last count we have all helped to raise over £7,000! Thank you for your help. This piece was the first of our Pocapalooza specials; “Poc” standing for “perioperative care” and “palooza” meaning festival. This piece is an update on The African Surgical Outcomes Study (ASOS) and the African Perioperative Research Group (APORG): “Everybody in the audience was overwhelmed by the quality and importance of the work being done … in the African study”; “Finding African solutions to the problems that we’re having” You will also find a discussion regarding the Prato conference we covered extensively here on TopMedTalk, a fascinating gathering of people interested in international collaborative trails, and we cover The UK Perioperative Medicine Clinical Trials Network, see link below, before moving on to ASOS in detail. The Perioperative Medicine UK Clinical Trials Network is here: https://pomctn.org.uk/home Presented by Desiree Chappell with Monty Mythen and their guests Paul Myles, Director, Anaesthesiology and Perioperative Medicine, Alfred Hospital and Monash University, Bruce Biccard consultant anaesthetist at the University of Kwazulu-Natal, Director, Perioperative Research Unit, Department of Anaesthetics, Nelson R Mandela School of Medicine and Salome Maswime Obstetrician and Gynaecologist and Professor of Global Surgery at the University of Cape Town.
