COVID 19 | Case study: Professor Monty Mythen 02

This is the second and concluding part of our real life case study of a COVID 19 patient. In this instance our Editor in Chief Monty Mythen has – during the course of presenting this series – contracted COVID 19. Fortunately for us, it was a comparatively mild case. This short documentary takes the story up right after Monty’s decision to take a test to see if his symptoms were as we feared. Presented by Nick Margerrison, part 1 is available of download here: https://www.topmedtalk.com/covid-19-case-study-professor-monty-mythen-01/ The longer conversations which these clips were part of can be found here: COVID 19 | Professor Monty Mythen has tested positive: https://www.topmedtalk.com/covid-19-professor-monty-mythen-has-tested-positive/ COVID 19 | Dr Joff Lacey: https://www.topmedtalk.com/covid-19-dr-joff-lacey/ COVID 19 | Special Charity Concert with Jeremy Sassoon 5pm UK time: https://www.topmedtalk.com/covid-19-special-charity-concert-with-jeremy-sassoon-5pm-uk-time/ Thank you to our sponsor Edwards Lifesciences. COVID 19 | Case study: Professor Monty Mythen 01

This piece looks at our recent archive and pieces together the story of our Editor in Chief, Monty Mythen’s bout of COVID 19. From the start TopMedTalk was keen to reflect the global battle against this terrible virus and – as you will hear – concerns for our friends and contributors in the medical world was never far from our minds. In retrospect it is particularly interesting listening to how evidence and best practice were in this instance vital to the health of our team and the people they care about. Presented by Nick Margerrison, this is part one of a two part series. The clips used in this piece are taken from fuller conversations which you can hear here: COVID 19 | Desiree and Monty: https://www.topmedtalk.com/covid-19-desiree-and-monty/ COVID 19 | Hugh Montgomery – North London UK: https://www.topmedtalk.com/covid-19-hugh-montgomery-north-london-uk/ COVID 19 | From the front line; Desiree and Monty: https://www.topmedtalk.com/covid-19-from-the-front-line-desiree-and-monty/ COVID 19 | UK, Southampton and London: Mike Grocott and Monty: https://www.topmedtalk.com/covid-19-uk-southampton-and-london-mike-grocott-and-monty/ COVID 19 | Does one of our team have COVID 19? https://www.topmedtalk.com/covid-19-does-one-of-our-team-have-covid-19/ COVID 19 | Critical Care Update; optimizing care for patient and provider: https://www.topmedtalk.com/covid-19-critical-care-update-optimizing-care-for-patient-and-provider/ Thank you to our sponsor Edwards Lifesciences. COVID 19 | An update on patient, Professor Monty Mythen

This piece is a reliable first hand account of COVID 19, directly from a patient, our Editor in Chief Monty Mythen. “This kind of reassurance that seemed to go with it, to seem to say to people: ‘nah, nah, nah, it’s ok; you’ve just got a bit of a sore throat and a bit of a dribbly nose, crack on!’ Was not supported by the available evidence at all” How is Monty feeling? What are the actual symptoms of COVID 19? Why has misleading information been apparently knowingly released regarding this? What are the details on The Nightingale Hospital in London? Further discussion includes best practice related to COVID 19 patient care including ventilatory strategies and fluid management. Previous information is reviewed in the light of new evidence. We also congratulate our partner in TopMedTalk Dr Henry Howe who has graduated today! Presented by Desiree Chappell with Monty Mythen. COVID 19 | Professor Monty Mythen has tested positive

“It is interesting how well you can feel, compared to the night time […] it gives you this real false sense of wellness for a period of time and then it hits you, man it hits you hard, when it hits you.” The UK Prime Minister has tested positive for COVID 19 as has The UK’s Health Secretary, Matt Hancock, MP. Yesterday we revealed there were concerns our Editor in Chief Monty Mythen had some of the symptoms, only days before he was set to return to active duty in Critical Care. Acting responsibly precautions were already being taken and the assumption was that it was likely, given its profusion in London where he lives, that he had the new virus. The test results confirmed that last night. Also on this podcast we get into further discussion regarding Continuous Positive Airway Pressure (CPAP), something which we have discussed a lot in this series, see links below. There is also an assortment of questions from the audience, if you’d like to add your thoughts to one of next week’s podcasts email us now: contact@topmedtalk.com Presented by Desiree Chappell with Monty Mythen, Editor in Chief of TopMedTalk. Dingle Highlight | Enhanced Recovery with Monty and Desiree

TopMedTalk has recently engaged with a large number of new listeners, we are delighted to have you join our family of perioptimists! This piece works as a primer, it explains what lies at the core of the world’s favorite perioperative medicine podcast; enhanced recovery. What is it? How do you implement it? What are its core principles? If you’ve never heard TopMedTalk before but you are a medical practitioner this piece is a great place to start; if you’re already onboard it will be a welcome refresher regarding the ideas we all gather around daily. How widespread is enhanced recovery after surgery and, perhaps more importantly, how widespread are its principles? Are patients living the drinking, eating and mobilising early on and so “living the (drink, eat, mobilise) dream?”. COVID 19 | Desiree and Monty

This week begins with an update on Monty’s progress after his COVID 19 diagnosis. Have the symptoms returned or is he back to his usual self? One thing we know for certain when it comes to this virus and our treatment of it’s symptoms; it is a dynamic situation. Management of the COVID patient is constantly changing. Do we treat our patients as having Acute respiratory distress syndrome (ARDS) or do we take a standard approach to ventilation? This piece looks at how our understanding of COVID 19 is developing as more evidence comes in. There then follows a conversation about the various uses medical research might be able to put Monty’s blood to, now he’s a recovering COVID 19 patient. Are there possible treatments on the horizon? Inevitably this requires evidence and rapid trials; “equipoise” is the word of the moment.
